CVE-2026-124476 in OpenEMRinfo

Summary

by MITRE • 11/08/2026

OpenEMR before 8.3.0 contains an insecure direct object reference in portal/lib/paylib.php, which writes the request-supplied form_pid (or the patient_id_cc value in Sphere data) to onsite_portal_activity without checking it against the session patient. Authenticated portal patients can submit portal-save, review-save, AuthorizeNet, Stripe, or Sphere requests to insert or modify forged payment-audit records in other patients' billing records.

Analysis

by VulDB Data Team • 11/08/2026

The vulnerability identified in OpenEMR versions prior to 8.3.0 represents a critical failure in access control mechanisms within the patient portal module. Specifically, this flaw manifests as an Insecure Direct Object Reference (IDOR), categorized under CWE-639, where the application fails to verify that the user is authorized to access or modify the specific data object requested. The technical root cause lies in the file portal/lib/paylib.php, which processes payment-related requests by accepting a form_pid parameter from the client-side request. Instead of validating this identifier against the patient ID associated with the currently authenticated session, the system directly utilizes the supplied value to write entries into the onsite_portal_activity database table. This lack of server-side authorization checks allows any authenticated user to manipulate billing records belonging to other patients by simply altering the pid parameter in their HTTP requests.

From an operational perspective, this vulnerability enables a wide range of malicious activities that compromise both data integrity and financial security within the healthcare environment. An attacker who has valid credentials for one patient account can exploit this flaw to forge payment audit records associated with different patient accounts. This capability extends across multiple integration points including portal-save, review-save, AuthorizeNet, Stripe, and Sphere request handlers. By injecting false transaction logs or modifying existing billing data, an adversary could potentially obscure fraudulent activities, create discrepancies in financial reporting, or disrupt the accurate tracking of healthcare services rendered to specific individuals. The impact is particularly severe given that OpenEMR is widely used for managing sensitive electronic health records where trust in the integrity of administrative and financial data is paramount.

This vulnerability aligns with several entries in the MITRE ATT&CK framework, most notably T1078 Valid Accounts, as exploitation requires initial authentication to the portal system. Furthermore, it relates to T1195 Compromise Infrastructure if an attacker leverages this flaw to establish persistence or cover their tracks within the hospital's information systems. The ability to modify billing records without authorization also touches upon data manipulation tactics that can lead to significant regulatory compliance issues under standards such as HIPAA in the United States, which mandates strict controls over protected health information and its associated administrative processes.

To mitigate this risk, organizations running OpenEMR must upgrade immediately to version 8.3.0 or later where these access control checks have been implemented. In addition to upgrading, administrators should enforce strict input validation on all server-side endpoints that handle patient identifiers, ensuring that any pid parameter is cross-referenced against the session's authenticated user context before processing. Implementing comprehensive logging and monitoring for unusual patterns in billing activity can also help detect potential exploitation attempts early. Regular security audits focusing on object reference validations across all portal modules are recommended to prevent similar vulnerabilities from being introduced during future development cycles.
