The issue has been introduced in 06/01/1993. The weakness was disclosed 11/01/1997 as 19971103-01-PX as not defined advisory (Website). The advisory is shared at patches.sgi.com. This vulnerability is known as CVE-1999-0328. The exploitation appears to be easy. An attack has to be approached locally. The exploitation doesn't need any form of authentication.
