A vulnerability, which was classified as critical, has been found in OpenEMR up to 8.0.0.2. This issue affects some unknown processing of the component DICOM Export. The manipulation with an unknown input leads to a path traversal vulnerability. Using CWE to declare the problem leads to CWE-22. The product uses external input to construct a pathname that is intended to identify a file or directory that is located underneath a restricted parent directory, but the product does not properly neutralize special elements within the pathname that can cause the pathname to resolve to a location that is outside of the restricted directory. OpenEMR is a free and open source electronic health records and medical practice management application. Prior to 8.0.0.2, the DICOM zip/export feature uses a user-supplied destination or path component when creating the zip file, without sanitizing path traversal sequences (e.g. `../`). An attacker with DICOM upload/export permission can write files outside the intended directory, potentially under the web root, leading to arbitrary file write and possibly remote code execution if PHP or other executable files can be written. Version 8.0.0.2 fixes the issue.

The advisory is shared at github.com. The identification of this vulnerability is CVE-2026-25928 since 02/09/2026. The exploitation is known to be easy. The attack may be initiated remotely.
