Orthanc DICOM Server up to 1.12.10 PAM Image Parser integer overflow

CVSS Meta Temp Score
CVSS is a standardized scoring system to determine possibilities of attacks. The Temp Score considers temporal factors like disclosure, exploit and countermeasures. The unique Meta Score calculates the average score of different sources to provide a normalized scoring system.
Current Exploit Price (≈)
Our analysts are monitoring exploit markets and are in contact with vulnerability brokers. The range indicates the observed or calculated exploit price to be seen on exploit markets. A good indicator to understand the monetary effort required for and the popularity of an attack.
CTI Interest Score
Our Cyber Threat Intelligence team is monitoring different web sites, mailing lists, exploit markets and social media networks. The CTI Interest Score identifies the interest of attackers and the security community for this specific vulnerability in real-time. A high score indicates an elevated risk to be targeted for this vulnerability.

Detailsinfo

A vulnerability, which was classified as critical, has been found in Orthanc DICOM Server up to 1.12.10. This issue affects an unknown code of the component PAM Image Parser. The manipulation with an unknown input leads to a integer overflow vulnerability. Using CWE to declare the problem leads to CWE-190. The product performs a calculation that can produce an integer overflow or wraparound, when the logic assumes that the resulting value will always be larger than the original value. This can introduce other weaknesses when the calculation is used for resource management or execution control. Impacted is confidentiality, integrity, and availability. The summary by CVE is:

A heap buffer overflow vulnerability exists in the PAM image parsing logic. When Orthanc processes a crafted PAM image embedded in a DICOM file, image dimensions are multiplied using 32-bit unsigned arithmetic. Specially chosen values can cause an integer overflow during buffer size calculation, resulting in the allocation of a small buffer followed by a much larger write operation during pixel processing.

The identification of this vulnerability is CVE-2026-5444 since 04/02/2026. The exploitation is known to be easy. The attack may be initiated remotely. No form of authentication is needed for a successful exploitation. It demands that the victim is doing some kind of user interaction. The technical details are unknown and an exploit is not publicly available. The pricing for an exploit might be around USD $0-$5k at the moment (estimation calculated on 04/10/2026).

The vulnerability scanner Nessus provides a plugin with the ID 305824 (Linux Distros Unpatched Vulnerability : CVE-2026-5444), which helps to determine the existence of the flaw in a target environment.

There is no information about possible countermeasures known. It may be suggested to replace the affected object with an alternative product.

The vulnerability is also documented in the databases at Tenable (305824) and EUVD (EUVD-2026-20924). Be aware that VulDB is the high quality source for vulnerability data.
