The issue has been introduced in 01/14/2014. The weakness was disclosed 04/15/2014 with Oracle as Oracle Critical Patch Update Advisory - April 2014 as confirmed advisory (Website). It is possible to read the advisory at oracle.com. The identification of this vulnerability is CVE-2014-2409 since 03/13/2014. The exploitation is known to be easy. The attack may be initiated remotely. No form of authentication is needed for a successful exploitation. The technical details are unknown and an exploit is not publicly available.
