The bug was discovered 03/09/2012. The weakness was published 03/21/2017 as HT207599 as confirmed advisory (Website). It is possible to read the advisory at support.apple.com. The identification of this vulnerability is CVE-2012-1147 since 02/14/2012. The attack may be initiated remotely. No form of authentication is needed for a successful exploitation. The technical details are unknown and an exploit is not publicly available. The advisory points out:

Multiple issues existed in expat. These issues were addressed by updating expat to version 2.2.0.

The vulnerability was handled as a non-public zero-day exploit for at least 104 days. During that time the estimated underground price was around $5k-$25k. The vulnerability scanner Nessus provides a plugin with the ID 59639 (SuSE 10 Security Update : expat (ZYPP Patch Number 8015)), which helps to determine the existence of the flaw in a target environment. It is assigned to the family SuSE Local Security Checks and running in the context l. The commercial vulnerability scanner Qualys is able to test this issue with plugin 370347 (Apple iTunes Prior to 12.6 Multiple Vulnerabilities (APPLE-SA-2017-03-22-1) and (APPLE-SA-2017-03-22-2)).

Upgrading to version 12.6 eliminates this vulnerability. A possible mitigation has been published immediately after the disclosure of the vulnerability.
